The following form is intended to help to address issues or impediments associated with fulfilling or completing projects funded through the American Recovery and Reinvestment Act of 2009 (federal stimulus). If you question relates to a project unrelated to the stimulus package please contact
the appropriate agency. Thank you!
To help the group address your concern more effectively, please:
- Fill out all of the fields provided in the form;
- Identify or indicate the location of the project, itself;
- Please provide as much information as possible about the project (i.e., permit #, project #, etc.), and the issue or impediment you require assistance with; and
- If the project is under the purview of multiple agencies, in addition to primary agency of relevance in the form, please identify those other agencies in the text field at the bottom of the form.
