Filling in a form
Information regarding the origin, types, amounts, and destinations of Connecticut solid waste disposed and recycled is essential for state and local solid waste planning, assessment, and compliance assurance.  The reports submitted to the Connecticut DEEP by municipalities, solid waste collectors (haulers), and solid waste facilities pursuant to state statutes, regulations, and solid waste facility permit requirements are the primary sources of this data  and are used for a variety of purposes
 
Forms for reporting this information will be posted below as their web versions are developed.   If the form for your facility type has not yet been posted on this webpage, please use the most recent copy of the reporting form already provided to you.

The reporting forms are designed to both provide essential solid waste information, and be convenient for users to complete. The following reporting forms (for solid waste facilities with individual permits) are available upon request.  Call Paula Guerrera (860) 424-3334 to request a copy of a reporting form.
NEW - Please be aware that starting with the third quarter 2016 (July 2016-September 2016) and going forward, quarterly solid waste and recycling facility reports submitted to DEEP will be able to report tonnages for the quarter and will no longer have to report separate tonnages by month.  If you have any questions, please contact Paula Guerrera (contact information above).
Composting Facilities

Owner or operator for each CT solid waste facility with a CT solid waste permit or registration to process only source separated organic material (e.g. leaves, grass, yard waste, wood, food waste, etc.)

Recycling Transfer Stations

Owner or operator for each CT regional or multi-town solid waste transfer station (TS) with a CT solid waste permit to transfer materials for recycling.
Resource Recovery Facility

Operation Quarterly Report
Owner or operator for each CT resource recovery facility with a CT solid waste permit to incinerate solid waste and produce energy.
Annual Reporting Form
All CT scrap metal processors.
Solid Waste Recycling Facilities
Quarterly Reporting Form 
(1) Owner or operator for each CT solid waste facility with a CT solid waste permit to process source separated recyclables; and
(2) Out-of-state recycling facilities which receive specific Connecticut generated recyclable items which did not pass through a CT permitted solid waste facility.
Solid Waste Wood Grinding Recycling Facilities 
Quarterly Reporting Form 
Owner or operator of each CT solid waste facility with a CT solid waste permit to only process and grind source separated untreated wood waste for recycling.  Wood burned for energy is considered disposed – not recycled.
Solid Waste Transfer Stations
Quarterly Reporting Form for regional or multi-town solid waste transfer stations.
Quarterly Reporting Form for municipal single-town solid waste transfer stations.
Owner or operator of each CT solid waste transfer station (TS) with a CT solid waste permit to transfer any type of solid waste for disposal.  
Tire Volume Reduction Facilities
Quarterly Reporting Form
Owner or operator of each CT volume reduction plant (VRP) with a CT solid waste permit to process only tires.
Visual Observation of Incoming Loads
To identify loads of solid waste (other than loads of source separated recyclables) containing >10% designated recyclables and loads of source separated recyclables containing >2% contamination. This information needs to be included in the facility’s quarterly SW reports submitted to DEEP.
Owner or operator of any CT solid waste or recycling facility required to track this information pursuant to permit requirements and/or pursuant to CT General Statutes requirements – e.g. CGS Section 22a-220c(b).
