If you elected a refund of your Supplemental and/or Voluntary account or used any of these funds to purchase additional service credit, an Application for Withdrawal of Supplemental and/or Voluntary is due on or before November 30th, 2009.  If your intent is to roll-over the taxable portion (if any) into another qualified plan and the application is not received by November 30, 2009, a refund will automatically be issued on December 30, 2009 with 20% of the taxable amount withheld for federal income tax purposes as required by the IRS.  You can access the application via the following link:
