If you elected a refund of your Supplemental and/or Voluntary account on your Application for Retirement Benefits and you have not submitted an  Application for Withdrawal of Supplemental and/or Voluntary, the form is due on or before November 30th, 2012.  If we have not received the application by November 30, 2012, we will issue a refund to you on December 28, 2012; report the taxable amount as taxable income to you; and withhold 20% of that amount for federal income taxes.
