2008-2009 Member Annual Statements

 
We will issue 2008-2009 Member Annual Statements in December, 2009. 
 
Are you a new teacher?  If you began teaching on or after July 1, 2009 you will not receive a 2008-2009 Member Annual Statement.  Your first statement will be for the 2009-2010 school year which will be issued in January, 2011.  All demographic changes/corrections (name, address, date of birth or social security number) must be done through your employing school district.  You should complete and forward an Active Teacher Beneficiary Form (PDF 107KB) directly to CTRB for processing. 

Active Teachers

Have you moved?  If you have changed your address, simply notify your employer.  It will be your employer's responsibility to notify us of the change via their next monthly transmittal report to CTRB.

Is your beneficiary election up to date?  Marriages, births, and other status changes may cause you to change your beneficiary designation.  To update your election, obtain an Active Teacher Beneficiary Form (PDF 107KB).  You should return this completed form directly to CTRB for processing.

Are you going on a Leave of Absence or considering one?  Obtain the Leave of Absence Bulletin (PDF 60KB) for specific instructions and documentation requirements.
