When to file a report?
 
Each agency's report is due no later than the 15th day of the month.
 

What should you file on a report?
 
Each agency's representative is responsible for running and reviewing contract data from the CORE-CT through a series of specially designed queries.  Each agency is then responsible for verifying the accuracy of CORE-CT based contract data.  By submitting SEEC Form 13, the agency is authorizing the SEEC to rely on the report for that month from CORE-CT (except as modified by information provided in Sections II, III and IV of the form) to publish the list of the contractors who are prohibited from making contributions to the committees.
 

How to: complete Section II?
 
You must complete this section if you have any of the following to report:
 
i) exclude entities/contractors included on the Core-CT Contract Report that do not meet definition of state contractor for the following reasons; municipality or agency thereof, exclusively federally funded, education loan or loan to individual for other than commercial purposes; or
 
ii) contractors whose contracts have expired or terminated, but not yet corrected in the Core-CT system; (Note that contractor should be included on list through 12/31 of the calendar year during which the contract was terminated.)
 
The Status line shows the filing status of the selected filing period for this section.  Before submitting this form this line is blank. 
Upon review of the CORE-CT report for your agency, if you do not have exclusions to be reported then click on the "I have nothing for this month". This option is the default action of section II. If you have exclusions to be reported then click on the "I am submitting a new file to replace last month's data" and then click on the Browse button.  Upload one of the recognized format files.
 
Note: you can download the exclusions file template found on TEMPLATE Tab of SCCBS web link or from the links at the bottom of section II on this form. 
 
Make sure to provide the following information:

1. FEIN:  This is required information.  You must provide the FEIN as reported in the CORE-CT system of the state contractor that you wish to exclude from the list.
2. Contractor Name:  This information is optional.  Provide the complete name of the state contractor as it appears on the CORE-CT report.
 
3. Reason for Exclusion:  This is required information.  The reason statement should not be more than 150 characters long.  You must provide a brief explanation for the exclusion.  The following are examples of exclusion reasons you may wish to use:
 
a. Mistakenly included on the CORE-CT report.
b. Not subject to Public Act 07-1
c. Contract has expired.
You can use the same file to report exclusions for any of the following filing periods, just add the new entries to the end of the existing file.  You must report information for exclusions on a cumulative basis for a given calendar year.
 

How to: complete Section III?
 
The Contract Report that SEEC receives from CORE-CT system captures only contracts in the CORE-CT system.  Agencies that have state contracts which are not reported in the CORE-CT system must provide a list of the names of those contractors whose contracts have a value of $50,000 or more, including all information requested in Section III of SEEC Form 13. 

NOTE:  This section is only completed by agencies that do not report their contract/expenditure information using the CORE-CT System. The Status line shows the filing status of the selected filing period for this section.  Before submitting this form this line is blank.   If you have no contractors to report to SEEC or you wish to use last month's data that you submitted, click on the "Use last month's data". This option is the default action of section III. To report the list of contractors, click on the "I am submitting a new file to replace last month's data" and then click on the Browse button.  Upload one of the recognized format file.
 

Note: you can download the file template found on TEMPLATE Tab of SCCBS web link or from the links at the bottom of section III on this form. 
 
Make sure to provide the following information:
1. FEIN:  This is required information.  You must provide the contractor's FEIN to the extent known.  FEIN helps identify uniquely contractors and verify that the list posted on the SEEC website is accurate.

2. Contractor Name:  This is required information.  You are allowed 150 characters for the contractor name.  You must provide the complete name of the State Contractor as it appears on the contract.

 
3. Contact Name:  This is optional information.  You are allowed 100 characters for the contractor contact name.  You may provide the complete name of the contact person for the state contract.  Use First Name, Middle Initial/Name and Last Name format for the contact name (for example, John A Doe or John Andrew Doe).

4. Contact E-Mail Address:  This information is optional.  Provide the email address of the contact person.  You are allowed 100 characters for the email address.  You must use username@domain.ext format for the email address (for example, john.doe@ct.gov).

5. Contract Start Date:  This is required information.  You are allowed 10 characters for this information.  You must provide the date the contract or agreement has been entered into.  Use mm/dd/yyyy format for the start date.

6. Contract End Date:  This information is optional.  You are allowed 10 characters for this information.  You must provide the date the contract or agreement is to end.  Use mm/dd/yyyy format for the end date, if any.  If this information is unknown then leave it blank.
You can use the same file to report state contractors for any of the following filing periods, just add the new entries to the end of the existing file.  You must report information for state contractors on a cumulative basis for a given calendar year.
 
If you wish to remove the contractors you reported to SEEC in previous month, click on the "Delete last month's data" option.

 
 How to: complete Section IV?

Complete this section if the agency is reporting names of Prospective State Contractors that have submitted a response to a state contract solicitation (including bids, RFPs or RFQs), or hold a valid prequalification certificate issued by the Commissioner of the Department of Administrative Services. 
 

NOTE: Prospective State Contractors are not reported in the CORE-CT system; therefore agencies with prospective state contractors are responsible for reporting this information to the SEEC. The Status line shows the filing status of the selected filing period for this section.  Before submitting this form this line is blank.  To report the list of prospective contractors, click on the "I am submitting a new file to replace this month's data" and then click on the Browse button.  Upload one of the recognized format file.
 
Make sure to provide the following information:

1. FEIN:  This is required information.  You must provide the prospective contractor's FEIN to the extent known.  FEIN helps identify the uniquely contractors and verify that the list posted on the SEEC website is accurate.
2. Contractor Name:  This is required information.  You are allowed 150 characters for the contractor name.  You must provide the complete name of the Prospective Contractor as it appears on the contract solicitation.

3. Contact Name:  This is required information.  You are allowed 100 characters for the contact name.  You must provide the complete name of the contact person for the bid or proposal.  Use First Name, Middle Initial/Name and Last Name format for the contact name (for example, John A Doe or John Andrew Doe).

4. Contact E-Mail Address:  This information is optional.  Provide the email address of the contact person.  You are allowed 100 characters for the email address.  You must use
username@domain.ext format for the email address (for example, john.doe@ct.gov).
5. State Contract Solicitation Period: 

NOTE:  Not applicable to holders of valid prequalification certificates issued by the Department of Administrative Services.  You are allowed 100 characters for this information.  Provide the beginning and ending date of the contract solicitation period.  The contract solicitation period is the date the RFP, bid solicitation and/or response was submitted until the date the contract has been entered into, or the notification date of the non-awarding of the contract. Use mm/dd/yyyy format for the dates. If this information is unknown, leave it BLANK.  If a state agency does not report its contract information in the CORE-CT system and a contract has been entered into as a result of solicitation, the contracting agency will remove the name of  "prospective state contractor" from Section IV and report the state contractor in Section III.  In addition, if a contract is not awarded, the prospective state contractor? is removed from Section IV and not reported in any other section.  You can use the same file to report prospective state contractors for any of the following filing periods, just add the new entries to the end of the existing file.  You may report information for the prospective state contractors on a cumulative basis for a given calendar year.

How to make corrections to already filed form?

You may re-file this form with correction to one or all sections of this form, as many times as necessary, as long as you re-submit before midnight of the 15th of each month.
