NATIONAL GUARD HISTORICAL RECORDS
Request For and/or Release of Records
Procedures for obtaining DD Form 214 and/or other documents needed for verification of military service with the Connecticut National Guard:
1. Service member must submit a completed Standard Form 180, either through regular mail, e-mail, or fax. (Please include a brief note indicatating what you need the documentation for, it will help us determine if we should include any other documentation you may need).
2. Pertinent documents available from the individuals file will be copied, stamped as certified copies, and mailed to their Home of Record.
