Connecticut Home Care Program for Elders (CHCPE)

This website area explains the CT Home Care Program for Elders (CHCPE), including who is eligible for the program, what services are available, how to apply for services, and how to contact the program. 

To be eligible, applicants must be 65 years of age or older, be a Connecticut resident, be at risk of nursing home placement and meet the programís financial eligibility criteria. To be at risk of nursing home placement means that the applicant needs assistance with critical needs such as bathing, dressing, eating, taking medications, toileting.  The CHCPE helps eligible clients continue living at home instead of going to a nursing home. Each applicantís needs are reviewed to determine if the applicant may remain at home with the help of home care services.   For more information on eligibility criteria, please see the link below.

*Personal Care Attendant Services - family members are NOT eligible to be paid for taking care of relatives with very rare exceptions. 

*Chore Assistance - only available as part of a package of services to applicants who meet all other eligibility criteria.

Co-Pay Requirement

As of July 1, 2015 clients who are on the state-funded portion of the program are required to pay a 9% copay per Public Act 15-5.  The care manager assigned to you can explain the process in detail.  Failure to pay the copay will result in the termination of services under the CT Home Care Program for Elders.  

Change in State-Funded Category 1

Also as of July 1, 2015 enrollment of state funded category 1 clients will be frozen and those clients will be placed on a waiting list in case their needs increase to nursing home level of care in the future. Category 1 clients are those who are considered at risk but do not meet nursing home level of care.   

Assisted Living Demonstration Program residents (The Retreat), Hebert T. Clark, Smithfield Gardens and Luther Ridge) are excluded from copay requirements, per Section 17b-347e of the CT General Statutes and exempt from the category 1 waiting list.


Applied Income

If a clientís income is over the Medicaid limit, they may be required to contribute toward the cost of their care. If a clientís income exceeds the set amount, the client will be responsible for paying a monthly applied income amount. The  amount may be lowered with certain items submitted and taken into consideration. These are: paying from own funds for additional services, prescriptions, health insurance, and providing additional support to his/her spouse, medical bills.

The State of Connecticut wants to give you an opportunity to stay home instead of going to a nursing home. That is the purpose of the Home Care Program For Elders. Providers who wish to enroll as CHC Service Providers must be credentialed by Allied Community Resources (860-627-9500) and must fill out the enrollment application online at www.ctdssmap.com ---Provider---Enrollment.  Please read the instructions carefully before starting the enrollment wizard.  All required information must be submitted to complete the application. Paper enrollment applications are no longer accepted.  The provider manual can be accessed at www.ctdssmap.com ----Publications----Provider Manuals.  Information about changes for providers can be found on the home page of www.ctdssmap.com under Publications, Provider Bulletins and Important Messages.  Provider may also access free provider billing training sessions conducted by HP.
