DSS has begun a Primary Care Case Management pilot program for HUSKY A members. The option is now known as HUSKY Primary Care, Connecticut’s Primary Care Case Management (PCCM) program. In HUSKY Primary Care, HUSKY A members enroll with their primary care provider (PCP) directly, rather than with a HUSKY managed care health plan. 

This new health care delivery model is explained in the documents below. 

The program is currently available in the greater Hartford, New Haven, Waterbury and Windham/Willimantic areas. DSS will soon be expanding the program to the Putnam area (July 2010) and Torrington area (October 2010). The Department is seeking applications from primary care providers in all of these areas who are interested in joining the program.
