HUSKY Primary Care (PCCM): Providers Invited

DSS has begun a Primary Care Case Management pilot program for HUSKY A members. The option is now known as HUSKY Primary Care, Connecticut’s Primary Care Case Management (PCCM) program. In HUSKY Primary Care, HUSKY A members enroll with their primary care provider (PCP) directly, rather than with a HUSKY managed care health plan. 

This new health care delivery model is explained in the documents below. 

The program currently includes primary care providers in the greater Waterbury and Windham areas.  Later this year, the Department will be expanding the program to the greater New Haven and Hartford areas. The Department is seeking applications from primary care providers in these areas who are interested in joining the program.
Primary care providers who are interested in participating should refer to the letter to providers, provider application, and provider agreement below.
