My Community Choices

Options for Leaving a Nursing Facility
 and Moving to the Community

‘My Community Choices’ is a public information service that provides options for nursing home residents about returning to the community. You’ll find:

    • Information to assist in making an informed choice about where to live;
    • Assistance with finding a home in the community;
    • Materials designed to help residents be part of their move;
    • Opportunities to talk with someone who had similar life experiences;
    • One-to-one assistance to help plan and implement a move to the community;
    • Help with cooking, cleaning, chores and home health needs.

The design and development of "My Community Choices" is funded by a $800,000 grant over three years from the federal Centers for Medicare and Medicaid Services, administered by the state Department of Social Services. DSS subcontracted the project to the Connecticut Association of Centers for Independent Living, which works with five community-based independent living centers.

Independent living centers are organizations designed and operated within a local community by individuals with disabilities. They provide an array of independent living services. You’ll find referrals to independent living centers and other sources of assistance in the materials below.
