Find the forms you need:

  • Choose Current year forms or Past years forms, and select Forms by By tax type.

Important notes:

  • Our forms are not available in fill-in format, unless clearly indicated with the word (Fillable) next to the title of the form. You must use the Taxpayer Service Center to file your forms online.
  • All forms and some DRS publications are in Adobe Acrobat format. Before viewing or printing these documents you will need Adobe Acrobat Reader 7.0 or higher. For additional assistance or if you are have trouble downloading a form, visit our Adobe Information page.
  • Some of the forms and instructions on this Web site may not have been updated to accommodate the recent legislative changes.
  • Washington DC will celebrate Emancipation Day on Friday, April 15, 2016.  As a result, any federal return otherwise due on April 15 that is filed (mailed or transmitted) on Monday, April 18, 2016, will be considered timely filed.   Emancipation Day is considered a legal holiday for Connecticut income tax purposes.  Therefore, Connecticut income tax returns and payments normally due on April 15 will be treated as timely if they are filed and payments made on or before Monday, April 18, 2016.  Such returns include 2015 calendar year filers of Forms CT-1040, CT-1040NR/PY, CT-1040EXT, CT-1041, CT-1041EXT the first installment of 2016 Forms CT-1040ES and CT-1041ES, CT-1065/CT-1120SI, CT-1065/CT-1120SI EXT.
    • This change does not affect any other taxes, except use tax and gift tax.
