Conn. Agencies Regs. Sec. 12-293a(c)-1. Distributor's customer lists.
(a) Each distributor, as defined in section 12-285(a)(2)(B) of the Connecticut General Statutes, shall, on or before the first day of August of each year, file with the Department form AU-758, Cigarette Distributor's Annual List of Customers, enumerating the name, address, Connecticut tax registration number and Connecticut cigarette dealer license number of every dealer who is a customer of such distributor as of the last day of the next preceding June.
(b) Each distributor, as defined in section 12-285(a)(2)(B) of the Connecticut General Statutes, shall, on or before the tenth day of each month, file with the Department form AU-759, Cigarette Distributor's monthly list of customer changes, enumerating the name, address, tax registration number and cigarette dealer license number of every dealer who became a customer of such distributor or who ceased to be a customer of such distributor during the next preceding month. Form AU-759 shall be filed even if no dealer became a customer of such distributor during such preceding month or even if no dealer ceased to be a customer of such distributor during such preceding month, or both.
