Repealed Effective 12/5/03

Conn. Agencies Regs. Sec. 12-313-2a.  Forms, affidavits and applications.

(a) The following forms, affidavits and applications shall be furnished by the Department of Revenue Services, Audit Division, Excise Tax Subdivision:

(1) the annual list referred to in section 12-293a-1 (a) and known as Form AU-758.

(2) the monthly list referred to in section 12-293a-1 (b) and known as Form AU-759.

(3) the affidavit referred to in section 12-293a-2 and known as Form CT-33.

(4) the affidavits referred to in section 12-313-4a and known as Form CT-30.

(5) the form referred to in section 12-313-9a and known as Form CT-31.

(6) the form referred to in section 12-313-12a and known as Form AU-761.

(7) the monthly report referred to in section 12-293a of the general statutes and known as Form CT-15.

(b) The following forms, affidavits and applications shall be furnished by the Department of Revenue Services, Operations Division, Registration Section:

(1) the order form referred to in section 12-313-3a (d) and known as Form 0-252.

(2) the order form referred to in section 12-313-10a (a) and known as Form 0-252.

(c) The following forms, affidavits and applications shall be furnished by the Department of Revenue Services, Operations Division, Processing Section, Forms Unit:

(1) the dealer's license application referred to in section 12-287 of the general statutes and known as Form REG-2 CIG.

(2) the distributor's license application referred to in section 12-288 of the general statutes and known as Form REG-2 CIG.

(3) the monthly report referred to in section 12-293a of the general statutes, require from owners or operators of more than five cigarette vending machines, and known as Form OP-182.

(4) the semiannual report referred to in section 12-293a of the general statutes, required from owners or operators of one or more, but less than Six, cigarette vending machines, and known as Form OP-182.

(d) This section is prescribed pursuant to 12-313 of the general statutes.
