OHCA requests that all Freedom of Information (FOI) requests are submitted to OHCA on the FOI Request Form or the Hospital Inpatient Discharge Data Request Form.  

 

Copies of both FOI Request Forms can be obtained through OHCA’s web site or by Clicking Here.

 

In order to assist OHCA in expediting your request, please include the following information on the FOI Request form:

  • Requested Docket Number(s), if applicable.
  • Specifics as defined on the Request Forms(i.e. CON Decision or the CON application, Hospital Audited Financial Statements, etc.)
  • Your Name, Company Name, Address, Telephone number, Fax number and if applicable your email address.

Note:  Before submitting your request, please be sure to search our website to see if any of the information you are interested in is listed there.  This may save you time and money.

 

Requesting FOI or Questions:

 

ALL FOI Requests should be emailed to: dph.communications@ct.gov.  

 

OHCA assesses the following charges relating to FOI items:

  • $0.25 per paper page for copying document;
  • Charges for files provided on a USB will vary, please inquire with FOI staff;
  • A requestor may also be responsible for any applicable postage and shipping charges
  • Certain FOI items may require different charges depending on labor, cost to the agency to have obtain/convert the information request etc.   Please contact the agency regarding possible charges before submitting the request if the request is unusual. 
  • Any charges related to Hospital Inpatient Discharge Data, will be assessed after OHCA reviews the request.

Information Dissemination:

 

OHCA will make every effort to respond to your request in a timely manner.  However, if several requests have been received before yours and/or your request is voluminous, it may take OHCA some time to process the request. If that is the case, OHCA will contact you within four business days to let you know the status of your request. 

 

For Hospital Inpatient Discharge Data requests it generally takes a minimum of two to three weeks for OHCA to provide requested information.  Once a request is received, OHCA staff evaluates it for adherence to the confidentiality provisions and suppression thresholds of our regulations. The requester is notified within four business days of OHCA’s receipt of the request whether or not the request seeks confidential data elements or does not meet the thresholds outline in the agency’s regulations. All requests for data are then publicly noticed. The public has ten business days to comment on a noticed request with respect to its conformance to the agency’s confidentiality and threshold standards. Data requests cannot be released before the end of the notice and comment period.
 

Most FOIs will be completed by emailing information requested to the requestor. Larger files provided in Adobe (PDF) format will be distributed on USB drive.  These USB’s can either be requested for pick-up at OHCA’s Front Desk or they can be mailed out.

 

NOTE:   If you request a CON docket file and it is available as an electronic file, it will be provided to you on a USB drive unless specifically requested otherwise.

NOTE :      Please be sure to include a copy of the completed FOI request form.  If the charge is estimated to be over $10.00, prepayment may be requested.
