Outpatient Surgery Data Reporting:

  • NEW!!! Outpatient Surgery Data Reporting is due by January 2nd and July 1st annually.
  • OHCA will post a notification on this webpage when the data is available for freedom of information requests.
  • Complete and useable data, for encounters that occur between 10/1/2015 and 3/31/2016, is due by July 1, 2016. Updated information on who is required to report and reporting process guidelines are now available at Outpatient Surgery Data Reporting. Email related questions to HelpDesk.dph@ct.gov using the subject line: Outpatient Surgery Data Reporting.

Latest CON Deemed Complete:
  • On May 10, 2016, OHCA deemed complete Yale New Haven Health Services Corporation’s (“YNHHSC”) and Lawrence + Memorial  Corporation’s (“L+M”) application to transfer ownership of L+M to YNHHSC,  docket number 15-32033-CON.
  • On May 10, 2016, OHCA deemed complete Northeast Medical Group’s (“NMG”) and L&M Physician Association’s (“L&MPA”) application to transfer ownership of a group practice by merger of L&MPA into NMG , docket number 15-32032-CON.
  • On April 19, 2016, OHCA deemed complete Hartford Hospital’s application acquire a computed tomography and a 3T magnetic resonance imaging scanners, under docket No. 15-32062-CON.
  • On April 5, 2016, OHCA deemed complete Saint Mary’s Hospital and Trinity Health’s application, docket number 15-32045-CON, to transfer ownership of Saint Mary’s Hospital to Trinity.

  • On April 25, 2016, OHCA received a Certificate of Need application requesting authorization to transfer ownership of Franklin Medical Group, P.C. from St. Mary's Health System Inc., to Trinity Health-New England, Inc., Docket Number 16-32077-CON.

  • On April 12, 2016, the Office of Health Care Access received a request for a CON Modification from Medical Specialty Group, P.C.   • P.A. 15-146 Section 34 Certificate of Need Requirements Report, to the joint standing committee of the General Assembly on matters relating to public health, provides information on the requirements and expedited review process for certain certificate of need applications.

  • On February 19, 2016, OHCA received Prospect Medical Holding, Inc’s application to acquire Eastern Connecticut Medical Professional Foundation, Inc, under docket number 16-32067-CON.

  • On February 17,  2016, OHCA deemed complete Yale-New Haven Hospital’s application for the termination of the Occupational Medicine and Wellness Services located at 317 Foxon Road in East Haven and at 84 North Main Street in Branford under docket number 15-32040-CON.

  • On February 17,  2016, OHCA deemed complete Yale-New Haven Hospital’s application for the termination of the Outpatient Rehabilitation Services located at 84 North Main Street in Branford under docket number 15-32041-CON.

  • P.A. 13-208 Sec. 69 Cardiac Transfers Report (revised) – This is a report to the joint standing committee of the General Assembly on matters relating to public health.  The document provides information on cardiac patient transfers from Connecticut hospitals permitted by certificate of need to provide emergency/primary coronary angioplasty services only to other hospitals.
