CON Portal:
  • ATTENTION: Beginning July 28, 2017, all CON Application, CON Determination and CON Modification material can be submitted electronically to OHCA, via OHCA’s new CON Portal.

    The $500 CON Fee should also be filed with OHCA at the time of the submission of the CON Application, using MasterCard or Visa, through the CON Portal.


    Important: For facilities and health care systems registering for the CON Portal, please be sure that the person registering (registrant) for the portal is responsible for submitting and receiving all CON applications and follow up material (they will essentially be the sole contact person for OHCA on the submitted proposal), such as CON applications, Completeness Letters and Responses, Final Actions and any other CON Application related material. Only the registrant submitting the CON material will be able to submit follow up material and receive correspondence from OHCA regarding that project.

Outpatient Surgery Data Reporting:
  • NEW!!! Outpatient Surgery Data Reporting is due by January 1st and July 1st annually.
  • OHCA will post a notification on this webpage when the data is available for freedom of information requests.
  • Complete and useable data, for encounters that occur between 4/1/2017 and 9/30/2017, is due by January 1, 2018. Updated information on who is required to report and reporting process guidelines are now available at Outpatient Surgery Data Reporting. Email related questions to HelpDesk.dph@ct.gov using the subject line: Outpatient Surgery Data Reporting.

  • On August 29, 2017, OHCA deemed complete St. Vincent’s Medical Center application seeking authorization to terminate Primary Care Clinic services at the Family Health Center; Docket Number: 17-32165-CON.
  • On August 25, 2017, OHCA deemed complete New Era Rehabilitation Center, Inc.’s,  application to establish a psychiatric outpatient and mental health day treatment clinic for adults in New Haven, docket number 17-32150-CON.

  • On June 29, 2017, OHCA received from Western Connecticut Orthopedic Surgical Center, LLC and SCA-Western Connecticut, LLC, an application for the transfer of ownership of 11% of Western Connecticut Orthopedic Surgical Center, LLC ownership interest to SCA-Western Connecticut, LLC. The CON was assigned Docket Number 17-32176-CON.
