CONNECTICUT IMMUNIZATION PROGRAM
The mission of the Immunization Program is to prevent disease, disability and death from vaccine-preventable diseases in infants, children, adolescents and adults through surveillance, case investigation and control, monitoring of immunization levels, provision of vaccine, and professional and public education.

*NEW* DPH Immunization Program Provider Survey The Department of Public Health (DPH) Immunization Program is moving forward in its efforts to establish electronic reporting of immunizations data between provider health record systems (EHRs) and the Connecticut Immunization Registry and Tracking System (CIRTS). To be able to assess provider qualification and readiness for interoperability, we ask vaccine providers to complete the survey accessed through this link.
