for Certificates Purchased from the State or Town

Please Note:  Personal Checks are not accepted.

There is a $20.00 fee for each Birth Certificate requested from the town or city of birth, and a $15.00 fee for the Certification of Birth (wallet size). Remit a money order made payable to the town or city where the birth occurred.
 
There is a $30.00 fee for each Birth Certificate (or Certification of Birth) requested from the State Vital Records Office at the Department of Public Health.  Remit a postal money order or bank check made payable to the “Treasurer, State of Connecticut.”   
 
There is a $30.00 fee for each Certification of Birth Registration or Certification of Foreign Birth of persons born out of the country and adopted by CT residents.  Remit a postal money order or bank check made payable to “Treasurer, State of Connecticut.”
 
There is a $20.00 fee for each Death Certificate requested at the state or town/city level.  Remit a money order made payable to the town or city where the death occurred.  If you are sending your request to the State Vital Records Office, remit a postal money order or bank check made payable to “Treasurer, State of Connecticut.”  
 
There is a $30.00 fee for each License to Marry.  Payment must be submitted to the town at the time the marriage license is issued.
 
There is a $20.00 fee for each certified copy of a filed Marriage Certificate requested at the state or town/city level.  Remit a money order made payable to the town or city where the marriage took place. If you are sending your request to the State Vital Records Office, remit a postal money order or bank check made payable to “Treasurer, State of Connecticut.”
 
There is a $20.00 fee for each copy of filed Civil Union Certificate requested at the state or town/city level.  Remit a money order made payable to the town or city where the civil union took place. If you are sending your request to the State Vital Records Office, remit a postal money order or bank check made payable to “Treasurer, State of Connecticut.”
 
There is a fee of $30.00 for each Certificate of Stillbirth that is requested. Please remit a postal money order or bank check made payable to “Treasurer, State of Connecticut”.
