Comprehensive State Health Plans and Assessments

 
The Department of Public Health is the lead agency for public health planning in Connecticut. It is mandated to prepare a multi-year state health plan that provides an assessment of the health of Connecticut's population and the availability of health facilities. (CGS Sec. 19a-7.) 
 
Beginning in 2012, DPH, in collaboration with community partners from across the state, is undertaking a major initiative to develop a comprehensive State Health Assessment and State Health Improvement Plan. The activities are part of the National Public Health Improvement Initiative, and will fulfill key prerequisites for national accreditation.

A comprehensive plan covering the health of Connecticut residents, the state’s health care delivery system, the need for health services and programs, and their fiscal implications. Modeled on the national Healthy People initiative, set more than 100 health status and risk reduction objectives for the state in 17 priority areas.
A supplement to the original document, adding two “services and protection” priority areas  with objectives aimed at increasing the accessibility and quality of preventive care services.
