DPH: AEMT Certification by Examination

AEMT Certification by Examination

Before applying for certification, please familiarize yourself with the general certification policies.

An AEMT applicant must meet one of the following requirements:

  • Hold current Connecticut EMT certification at the time of enrollment and successful completion of a Department-approved 85 or 99 Advanced Emergency Medical Technician (AEMT) training program in Connecticut with a minimum of 100 hours that included written and practical examinations.

Documentation Requirements:

  • Download and complete an application for initial AEMT certification. PLEASE NOTE: this application is a fillable pdf.  In order for all the features to be fully functional, click the link to open the file, save it to your computer, then open it in Adobe Acrobat Reader.
  • An AEMT Training Program Completion Form, which is submitted directly to the Department by your instructor.
  • If under 18 years of age, a completed Parental Consent Form.
  • If applicable, a completed Verification of EMS Certification/Licensure Form from all states in which the applicant is or has ever been certified, current or expired forwarded to this office from the appropriate authority in any U.S. State or territory in which the applicant is or has ever been certified as a EMS provider.  Most jurisdictions charge a fee for verification.  Be sure to contact the jurisdiction for fee information.

An important note on certification issue dates:  The issue date of a new or reinstated (due to expiration) certification is based on the day the course was completed.  They are valid for a period not to exceed three years, and expire on the first calendar day of the quarter following the issue date (January 1st, April 1st, July 31st, October 1st).  Example: a certification issued on January 23rd, 2017 would have an expiration date of April 1st, 2020.

All supporting documentation should be sent from the primary source (state, territory, instructor).
