DPH: Reinstatement of a Lapsed Practical Nurse License

Reinstatement of a Lapsed Practical Nurse License

Before applying for licensure, please familiarize yourself with the general licensing policies.

In order to reinstate a license that has lapsed due to nonrenewal, an applicant must arrange for the submission of the following:

A completed application form and fee of $150.00 in the form of a bank check or money order (no personal checks will be accepted) payable to, Treasurer, State of Connecticut”.

A brief written summary of your professional activities since graduation from nursing school, to include work history (positions, dates, duties), any lapses in active practice as a nurse, and if not currently practicing as a nurse, an explanation of your plans for employment and describe how you intend to refresh your nursing skills; and

A letter submitted directly to this office from the appropriate authority verifying your most recent nursing experience confirming your employment, including dates and evaluation of clinical performance.  You may also use this form.

Please note that all reinstatement applications from nurses who have been out of the active, clinical practice of nursing for longer than three years and less than five years are required to complete an approved refresher course.  Applicants who have been out of practice longer than five years are required to complete an approved refresher course and successfully complete the NCLEX-PN examination.

Verification of all licenses held, current or expired forwarded directly from the appropriate authority in any U.S. state, territory or Canadian province in which the applicant is or has ever been licensed as either a practical nurse or a registered nurse.  Most jurisdictions charge a fee for completion of the verification form.   Please contact the jurisdiction for fee information.

The National Council of State Boards of Nursing's Nursys System verifies electronically licenses issued by several states.  Please select this link for a list of Nursys participating states. You may register for verifications for Nursys participating states online.

Temporary Permit

Applicants for reinstatement who hold a current, valid practical nurse license in another U.S. state or territory are eligible for a 120-day, non-renewable temporary permit to practice nursing in Connecticut pending licensure reinstatement.

If you have been out of the active clinical practice of nursing for a period longer than three years you are not eligible for temporary permit.  Once an application is complete, a temporary permit will be issued within 15 business days.
