Dental Hygienist Licensure Requirements

 

Before applying for licensure, please familiarize yourself with the general licensing policies.

 

An applicant for licensure must meet the following eligibility requirements:

 

Graduated from an American Dental Association (ADA) accredited dental hygiene program;

 

Successfully complete the National Board Examinations;

 

Successfully complete one of the following clinical performance examinations: 

 

The Commission on Dental Competency Assessments (formerly NERB);


Council of Interstate Testing Agencies;

 

Southern Regional Testing Agency;

 

Central Regional Dental Testing Service;

 

Western Regional Examining Board;

 

DOCUMENTATION REQUIREMENTS

 

Applicants should arrange for submission of the following documentation directly to this office from the source:

 

A completed application and fee in the amount of $150.00.  Applications are only accepted online.  Please select this link to apply online;

 

Official transcripts of dental hygienist education, verifying the award of the degree in dental hygiene;

 

Official report of National Board Dental Hygiene Examination scores. 

 

Official report of successful completion of a regional board examination as outlined above.

 

Verification of all licenses ever held current or expired from each state or territory in which the applicant is or has ever been licensed.  Most states charge a fee for completion of the verification form; contact each state or territory for fee information.
