DMV: Obtaining an SR-22 Financial Responsibility Form

Obtaining an SR-22
Financial Responsibility Certificate

An SR-22 is a Financial Responsibility Certificate that must be obtained through your insurance company or agent. It is insurance on your driver’s license and is separate from your standard automobile insurance policy.  The filing of this type of insurance certificate is required for the restoration of an operator's license that has been suspended for any number of serious violations.  This certificate must be filed by your insurance company before your operator license or operator license privilege can be restored.

To file an SR-22 with the Driver Services Division your insurance company must transmit the Certificate of Financial Responsibility to:

State of Connecticut
Department of Motor Vehicles
Driver Services Division
60 State Street
Wethersfield, CT 06161-2525

NOTE:  Neither facsimile nor photocopy submissions are acceptable.
