Plate requests are made on the Replacement Application Identical Plate Form E-45.  Below are the requirements and fees based upon the replacement plate need.

  • Mutilated Plate(s):

    Current issued plates that have been mutilated can be replaced as long as the registration is current.

    The fee is $25.

  • Lost or Stolen Plate(s):

    Applications to have a plate re-made are accepted 10 months from the date reported lost or stolen. However, that same plate can be released, if requested, to the general public one year after the date reported.

    You may only acquire the same plate type and special background that was reported lost or stolen.

    The registration type you are requesting must match the vehicle's current registration type. For instance, if you're requesting Combination plates, your vehicle must be currently registered with Combination plates. You cannot request Combination plates if your vehicle is currently registered with Passenger plate.

    The fee is $45.
