SERVICE UPDATE:  As DMV transitions to a new, online-only appointment system for road tests, DMV will stop taking road test appointments on October 21 and October 23, and will resume October 24 with its new convenient online system.  See more information here

ONCE APPROVED FOR THE ENDORSEMENT:
After presenting the documents at a branch office you will be issued a temporary paper card with the new endorsement on it.  This document should be kept with your current license until your new license arrives in the mail.  For more information on this, please see Central Issuance
 
 
Public Passenger Endorsements are no longer processed at DMV Hub Offices

What to do:

  1. Meet the eligibility requirements.
  2. Complete the Application for Public Passenger Endorsement (R-7).
  3. Complete the medical exam form (MCSA-5876).  The form must be dated within the last 90 days. Medical exams must be completed by an authorized medical examiner.
  4. Have your fingerprints (State Fingerprint Forms (DPS-125C) performed at: Any local/state Police Department or the state Department of Emergency Services and Public Protection headquarters. DO NOT BEND FINGERPRINT CARDS.
  5. complete a sex offender check through the national sex offender database. Website address: http://www.nsopw.gov. (If there are AKA's (other names used), all names must be run). The check submitted must have a date stamp from the printer it is printed from and must be within (5) days of the date of application.
  6. Connecticut resident less than 5 years: Must present a CERTIFIED driving and criminal history from each state you have lived in the past five (5) years. (AZ, CA, MS, NC, TN and VT are CLOSED record states and will only be required to submit a certified driving history.  Find your state and phone number to call here.
  7. S, V and A applicants only: Complete the Authorization for Release (Form DCF-3031) Important: This form will be sent DIRECTLY to Department of Children and Families (DCF) not to DMV.  Failure to send this form directly to DCF will result in a delay in the processing of your application.
What to submit:
Mail or drop-off the completed documents at the DMV Office in Wethersfield.  The office is open Tuesday, Wednesday and Friday from 7:45 a.m. to 4 p.m., and Thursday from 9:15 a.m. to 5:30 p.m.  There is a drop box near the 1st floor information desk. Applications will not be accepted at DMV branch offices.

  • Photo copy of valid Connecticut Driver's License
  • "S", "V" and "A" endorsement applicants only: Complete the Authorization for Release (Form DCF-3031)

  • $12.00 -- A signed personal check, bank check, and money order made payable to “DMV.”
