Case Management Services

CMHC offers a full range of community based clinical services to facilitate the matching of client needs with the appropriate level of service over time.  Services currently include the Outreach and Engagement Project, the Assertive Community Treatment (ACT) Team, and the Residential Team. These services are based at 235 Nicoll Street.

Goals and Objectives

  • To provide a flexible and responsive range of clinical case management services to individuals who have diagnoses of substance abuse, mental illness or both, and who have multiple unmet needs which have compromised their previous efforts to engage in conventional outpatient services.
  • To provide appropriate in-vivo rehabilitative and psycho-educational services to enable clients to develop the skills needed to successfully address their community living needs, including their needs for adequate food, clothing, shelter medical care, and meaningful activity.
