1. Follow the link to Registration Form to open a new window with the registration form.
2. Use computer to fill out fields:
a. For Checkboxes, you can use the “x” key to select or Left Click in
the box with your mouse.
b. Tab to move from one field to the next.
3. Save document to your computer {File>Save As…}.
4. Name your file something meaningful (e.g. JohnSmithUnderstPsychMeds3-1-09.doc).
5. If you want to fill out forms for multiple classes, go to B. Class Information section and remove the current class title, start date, sessions, and location and enter the new details.
6. Save the document again to your computer {File>Save As…}.
7. Name your next file something meaningful (e.g. JohnSmithCBT3-2-09.doc).
8. Repeat steps # 5-7 for as many registrations you would like to include.
9. In your email compose a new message to Education.Training@po.state.ct.us.
10. Please begin the subject line of the email with the words: “REGISTRATION FORM”
11. Attach your saved registration forms to your email message
12. Send the email.
