Status and Trends
Pursuant to Section 22a-39(m) of the General Statutes of Connecticut, and Section 22a-39-14 of the Regulations of Connecticut State Agencies, municipal inland wetlands agencies must complete this reporting form for each action taken by such agency.  Actions include but are not limited to: permit granted, permit denied, permit extended, approved map amendment, enforcement order, jurisdictional ruling, and agent approval of activity (see reporting form instructions for complete list of actions).
   The form and instructions are provided in two versions:
   1)  an electronic Word version with drop-down menus that can
        be saved to a computer; or
   2)  a PDF version that must be printed and completed by hand.
Whichever version is used, please remember to mail completed forms for all actions taken during a calendar month no later than the 15th day of the following month to the DEEP's Inland Wetlands Management Program.
