Obtain a Bingo Registration, Permit, and/or Personal Identification Number


First Time Bingo Applicants (For organizations that never conducted bingo in the past)

PLEASE NOTE:  If your organization has never conducted bingo in the past, it must complete a CGB-1, CGB-4 and CGB-4B application, a Bingo Price Sheet and a Bingo Prize sheet. Also, each of its volunteer members who will assist with the conduct of bingo must complete a CGB-2 and CGB/S-2A application. The person designated on the application for the bingo permit who will be responsible for the operation and conduct of the bingo games must review and be familiar with the provisions of the Connecticut General Statutes, the Administrative Regulations, Operation Of Bingo Games, and Public Act 03-178, as well as important policies regarding bingo. The designated Member In Charge of the bingo games should keep these laws, regulations and policies available for reference and use.

Previous Bingo Applicants - Class B ONLY

PLEASE NOTE:  If your organization has conducted bingo in the past, it should already be registered with the Department; therefore, it would only need to complete a CGB-4, and CGB-4B application for a bingo permit. New volunteer members who will assist with the conduct of  bingo, but have never applied for or received a Personal Identification Number (PIN) with your organization in the past, must complete a CGB-2 and CGB/S-2A application. The person designated on the application for the bingo permit who will be responsible for the operation and conduct of the bingo games must review and be familiar with the provisions of the Connecticut General Statutes, the Administrative Regulations, Operation Of Bingo Games, and Public Act 03-178, as well as important policies regarding bingo. The designated Member In Charge of the bingo games should keep these laws, regulations and policies available for reference and use.
