Complaint Forms and Procedures
Please send complaints to the Department of Consumer Protection in writing, preferably on one of the forms provided below. Include copies of all the appropriate documentation, and save copies of everything for your records. If you send a complaint by e-mail, we may need to contact you for further information. Please provide a daytime phone number and mailing address in your e-mail.

You may also e-mail your written complaint to dcp.frauds@ct.gov, but please be sure to include your street mailing address so that the processing of your complaint is not delayed.
