Policy
Team Review Process
The team review process encompasses
- the presentation of the child(ren)/ youth by the child(ren)’s Social Worker
- the discussion of the potential family resources.
Each family resource shall be discussed individually.
The child(ren)’s Social Worker and the FASU worker for the family shall
- be available to answer questions
- provide additional information as needed
- follow up with information needed to inform the discussion and decision making process.
Role of Child's Social Worker
Prior to the meeting the Social Worker shall:
- review all the home studies received from the Adoption Resource Specialist
- discuss the studies with his/her supervisor
- submit the names of the families for consideration to the team Chairperson
- complete the following forms:
- Permanency Planning Team Referral Form (DCF-2043)
- Child Teaming Pre-Questionnaire (DCF-2044)
- Child Assessment (DCF-2039)
- Genetic Parent Iinformation forms (DCF-337B and DCF-338B)
- submit the above forms to the team Chairperson
- notify FASU and private agency worker(s) whose families are being considered of the
- meeting date and time and invite them to attend, or be available by phone.
During the meeting the Social Worker shall:
- present information and respond to all questions from the team about the child
- indicate his/her choice of family(ies) resources for the child(ren) and the reasons for the decision.
Role of FASU Worker
The FASU Social Worker whose family (ies) is/are being considered shall be present at the team meeting or be available by phone to present the family and answer questions of the team.
Selection Process Criteria
The selection of a family resource shall be based on a comprehensive discussion of the history and needs of the child(ren), both present and anticipated, in concert with the history, experience, present circumstances and the child profile outlined by the prospective families.
Selection Process
The selection process is as follows:
- Each PPT member, including the child's Social Worker or Social Work Supervisor, shall identify the first and second choice of family and give reasons supporting his/her choice.
- The chairperson shall compile the data and identify the family that is the best possible match for the child(ren) being discussed.
- If possible, a second and third family may be identified as resources that will provide a good match for the child should the selected family be unavailable as the resource.
- In the event of a tie, the Social Worker and supervisor shall make the final decision taking into consideration the discussion of the team members.
Review of Families Not Selected
If...
a family has been identified as a prospective placement resource on six (6) different occasions
But...
has not been selected for any child
Then...
the home study will be reviewed by the assigned FASU and ARS Social Workers to identify areas of concern and determine if and how they can be addressed.
Appeal Process
If...
the Social Worker and Social Work Supervisor disagree with the decision of the team,
Then...
the Social Worker may appeal the decision to the Area Director within 48 hours of the team decision.
Appeals from any other party will be reviewed by the Area Director, at his or her discretion.
