AGING SERVICES: Medicare Open Enrollment ends December 7th

What is CHOICES?

Connecticut’s program for Health insurance assistance, Outreach, Information and referral, Counseling, Eligibility Screening.
Special Announcements and Events:
  • Medicare General Enrollment Period is January 1-March 31, 2018.  If you did not sign up for Medicare Part A and/or Part B during your Initial Enrollment Period, and don’t qualify for a Special Enrollment Period, then you may sign up at this time.  Your coverage will begin in July 1, 2018 and you may have to pay higher premiums due to enrolling late.  To sign up for Medicare, you may enroll online or go to your local Social Security Administration office.  To learn about Medicare benefits and programs that may help you lower your Medicare costs like the Medicare Savings Program and the Extra Help Low Income Subsidy, please contact CHOICES at 1-800-994-9422. 
  • Medicare Advantage Disenrollment Period is January 1-February 14, 2018.  If you’re enrolled in a Medicare Advantage Plan, you can dis-enroll and switch back to Original Medicare and join a Medicare Prescription Drug Plan during this time period.  Your coverage will begin the first day of the month after your new plan receives the enrollment request.  Please note during this period, you may not switch from Original Medicare to a Medicare Advantage Plan or from one Medicare Advantage plan to another one.  You are also not able to switch from one Medicare Prescription Drug plan to another one.  However, beneficiaries who have the Extra Help/Low Income Subsidy program can make changes to their Medicare plans throughout the year. The CHOICES Program: CT’s SHIP or State Health Insurance assistance Program
  • Is a cooperative program of the State of Connecticut Department on Aging, the Area Agencies on Aging and the Center for Medicare Advocacy, Inc.
  • Is funded by the Administration for Community Living, US Department of Health and Human Services and the Administration on Aging through the Older Americans Act. 
  • Produces several guides that can be found on the Aging Services Publication Page.
  • Is comprised of staff and in-kind professionals from local service agencies, and volunteers.
Provides the following types of information and assistance to individuals age 60 and older and all Medicare beneficiaries, including persons with disabilities:

  • Medicare Supplement
  • Medicare Advantage
  • Medicare Prescription Drug Plans
  • Medicare Savings Program
  • Extra Help/Low Income Subsidy
  • Medicare Preventive Services
Outreach presentations are conducted throughout the state of CT.  To attend, request, or schedule a presentation in your area, call 1-800-994-9422.  You may also contact your local CHOICES Regional Coordinator.  Outreach may include: 
  • Presentations: New to Medicare, Medicare Bootcamp, & CHOICES/SHIP
  • MIPPA eligibility screening & application assistance
  • Medicare Open Enrollment events
  • Booths at local senior & health fairs
Information and referrals to appropriate agencies which enables individuals to access needed services such as: 
  • Adult Day Care
  • Financial Assistance
  • Homecare
  • Housing
  • Nursing Home
  • Nutrition
  • Respite Care
  • Transportation
Eligibility Screenings and application assistance for federal and state benefit programs including: 
  • Supplemental Security Income (SSI)
  • Medicaid and Medicaid Waivers
  • Energy Assistance 
  • Caregiver Program
  • CT Home Care Program for Elders
**Eligibility determinations can only be made by the appropriate federal, state, or local agency managing the above programs.
